1 – 10 employees                     $   75.00 plus HST

11 – 50 employees                      150.00 plus HST

51 – 100 employees                    250.00 plus HST

101 + employees                         350.00 plus HST

Retired                                 50.00 plus HST
Student                                          20.00 plus HST
